Nurse Practitioner

The Lakeland Primary Care Network NPC is seeking a Nurse Practitioner (NP) with full time 1.0 FTE to provide services in the communities of First Nations and Metis Settlements within Treaty six of Lakeland PCN geographical area. Working to full scope of practice, this position will provide comprehensive nursing services within the primary care setting.


Successful completion of an approved Nurse Practitioner program at a Master of Nursing level or equivalent and active registration and practice permit with the CARNA as a Nurse Practitioner.
A minimum of two years of directly related experience in a primary health care, community health nursing with an emphasis on primary care services.  
This position requires a valid driver’s license and access to a motor vehicle in good repair for work related travel within the Lakeland Primary Care geographical area.
This position is community based.  Work may occur at various locations.
Current CPR certification. Current criminal record check. 
Computer skills required. EMR experience would be an asset.

Duties and Responsibilities: 

Participate in the care of individual patients, practice populations and associated communities. 
Make professional, competent and autonomous decisions with patients as part of a multidisciplinary team. 
Assist in the development and delivery of disease management and health promotion programs. 
Competent to perform comprehensive assessments, diagnosis (including the ordering and interpretation of tests), intervention, management, referral and evaluation of client care across the continuum of health care services. 
Utilize the nursing process, critical thinking, problem solving and decision-making skills to provide care to individuals, families, groups and communities. 
Contribute to quality collaborative inter professional patient care through advanced clinical practices, leadership, consultations, referrals and education. 
Foster an environment of empowerment engaging individuals, patients, families and the community in the planning, implementation and evaluation of health services. These areas include health promotion, illness/injury prevention, illness diagnosis and treatment, chronic disease management, senior’s health, mental health, rehabilitation and other related supports.
