REGISTERED NURSE. 0.5 FTE 

We are inviting applications for the Registered Nurse(RN) position at Dr.Johnson Fatokun, Smoky Lake Medical Clinic.

The Registered Nurse (RN) position would provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. The RN is responsible for assessment, management, follow up and support of clients, accurate charting and documentation, and collaborative work within the multidisciplinary team.
 
About the Lakeland Primary Care Network
Lakeland Primary Care Network is a growing charitable non-profit company serving seven communities in northeast Alberta delivering programs and services to enhance the delivery of primary health care. Our teams of nurses, social workers, dietitian, diabetes educator and physicians work together to provide excellent care for patients and their families.
 
Key responsibilities:
·  Work collaboratively within a family practice environment to enhance the delivery of primary care services.
·  Assessment of patient conditions, needs and goals in collaboration with the patient, physician and interdisciplinary team and provide appropriate follow-up.
·  Provide relevant patient education to prevent and manage clinical conditions.
·  Develop systems and communication protocols to facilitate patient follow up.
·  Assist in setting up health promotion programs relevant to the needs of the practice population.
·  Focus on health promotion and illness/injury prevention.
·  Assist in developing and implementing patient self-management programs.
·  Assist patients to navigate through the health system.
·  Communicate with patients to determine the next appropriate health services required based on presenting concerns and test results.
·  Monitor patient progress and evaluation of care using critical thinking, decisive action and resourceful problem solving.
·  Autonomous decisions in managing patients and complementing care provided by the physician.
·  Assist in completion of complex care plans with the primary care physician.
 
Qualifications:
·  Bachelor of Science in Nursing.
·  Current registration with the College & Association of Registered Nurses of Alberta.
·  Current health care provider CPR certification.
·  A minimum of 5 years of experience in nursing.
·  Excellent interpersonal, organizational and communication skills are required.
·  Ability to work cooperatively in a multi-disciplinary team environment.
