LICENSED PRACTICAL NURSE / REGISTERED NURSE

We are inviting applications for the Licensed Practical Nurse(LPN) / Registered Nurse(RN) position with 0.5 FTE at at Dr.Johnson Fatokun, Smoky Lake Medical Clinic.

 
The LPN position would support the family physicians in the daily delivery of primary care within a fast-paced family practice clinic setting, collaborating with PCN professionals, other local health care providers and community agencies as part of a patient-centered, multi-disciplinary health care team.
 
The Registered Nurse (RN) position would provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being.
 
About the Lakeland Primary Care Network:
Lakeland Primary Care Network is a growing charitable non-profit company serving seven communities in northeast Alberta delivering programs and services to enhance the delivery of primary health care. Our teams of nurses, social workers, dietitian, diabetes educator and physicians work together to provide excellent care for patients and their families.
 
Key responsibilities of RN:
·  Assessment of patient conditions, needs and goals in collaboration with the patient, physician and interdisciplinary team and provide appropriate follow-up.
·  Develop systems and communication protocols to facilitate patient follow up.
·  Assist in setting up health promotion programs relevant to the needs of the practice population.
·  Assist in developing and implementing patient self-management programs.
·  Assist patients to navigate through the health system.
·  Communicate with patients to determine the next appropriate health services required based on presenting concerns and test results.
·  Autonomous decisions in managing patients and complementing care provided by the physician.
·  Assist in completion of complex care plans with the primary care physician.
 
Qualifications for RN Position:
·  Bachelor of Science in Nursing.
·  Current registration with the College & Association of Registered Nurses of Alberta.
·  Current health care provider CPR certification.
·  A minimum of 5 years of experience in nursing.
 
Key responsibilities of LPN:
·  Works in conjunction with the physicians and/or independently as appropriate to assess the needs of patients; supports delivery of high quality, evidence-based health care to the practice population.
 
·  Conducts patients to consulting rooms, collects and enters patient vitals in EMR, and provides health services based on developed nursing knowledge and skills.
 
·  Works collaboratively to full scope of practice as a member of a healthcare team within a family practice environment to enhance the delivery of primary care services to patients.
 
·  Ensures clinic is stocked with medication and general medical supplies; maintains sterilization protocols, equipment standards, and supply levels of clinic exam rooms.
 
Qualifications for LPN Position:
·  Successful completion of an accredited Practical Nurse Diploma program.
·  Current health care provider CPR certification.
·  Minimum 2 years of experience in primary care including acute, community and continuing care.
